New Student Immunization Requirements

If you are a new student, whose program involves patient contact or exposure to human blood, tissue or cell lines, you are required to submit certain immunization records and to acquire any missing vaccinations before enrollment at UCSF.

You should have received a new student packet from Student Health and Counseling Services. If you have not received your packet, follow the steps below:

Step 1

Obtain the Immunization form for your entering quarter
Fall 2009

Step 2

Read the letter outlining the submission process
Fall Letter 2009

Step 3

Print out the Provider Letter to bring to your provider appointment to complete your pre-entry requirements.

Step 4

Complete your pre-entry requirements.

Step 5

Have your provider sign your form.

Step 6

Enter your data from the form into the secure Immunization Portal.

NOTE: Your SAA User ID and Password are needed to enter the site (this is the same User ID and Password you use for the SAA Student Portal).

Step 7

Make a copy of the signed form and mail the original to SHCS:

After you submit your form through the secure online portal, a Student Health RN will review your record. The nurse will contact you if there are any discrepancies.

Please be aware that you will not be allowed to register for classes or receive your financial aid disbursement until all pre-entry immunization requirements are met.
