ER/Urgent Care Claims

If you have recently visited an emergency room, an urgent care center or received a bill, make sure to follow these steps for continuity of care and insurance authorization.

Step 1

Contact the ER or urgent care center that you visited and give them a copy of your insurance information if you have not so already. Download your Member ID (on the My Health tab) or call 1-800-853-5899.

Step 2

Submit a Medical Records Release to the ER or urgent care center that you visited. In this release you want to authorize the ER or urgent care center to send your clinical (physician) notes from your visit to Student Health. These notes will be reviewed by Student Health.

Step 3

Call (415) 476-1281 and verify that SHCS have received your ER or urgent care visit notes.

NOTE: There is a $100 co-pay ER visits and a $200 policy deductible (for urgent care). For more information about your insurance see Medical Plan.
