**Update your gender identity and pronoun**
Please let us know your gender identity and pronoun. You can update this demographic information any time on the MyHealthRecord.ucsf.edu medical record portal under the PROFILE section on the left.
You may obtain a copy of your SHCS medical records at anytime. To request a copy of your medical records or to give another person access to your records, patients can submit an Authorization for Release of Medical/Billing Information online, by fax, by mail, or in-person. However, many of your records are already available on the MyHealthRecord Patient Portal and do not require prior authorization to view.
Records Available on the Patient Portal
Patients can access the records below without a written authorization by logging into MyHealthRecord Patient Portal:
- Lab test results
- Immunizations
- Referrals
- Letters
- Secure Messages

Immunization record:
You may obtain immunization records* at any time. The fastest, easiest way is to log on to MyHealthRecord, click on the “Immunization” section and print your own copy. If you are unable to access the records online, you may visit SHCS in person or call SHCS at (415) 476-6137 to request your records. There is no fee for printing immunization records.
*Students who opt to receive the COVID-19 vaccination may upload their vaccination record into the immunization records section in the MyHealthRecord Student Portal.Medical record: You may obtain a copy of your medical record at any time. Students may log on to MyHealthRecord to send a secure message and select the "Request Medical Records - Release of Information (ROI) eForm" option. If you are unable to access the records online, you may visit SHCS in person or call SHCS at (415) 476-6137 to request your records. Please note that obtaining medical records can take some time (up to three weeks).

If a healthcare organization is requesting your medical records, please call SHCS (415) 476-6137. There is no fee.

An insurance company may request a copy of your medical records when you apply for coverage. Your medical records cannot be released without your consent. If an insurance company requests a copy of your records, fill out their medical records release form and have them submit it to SHCS. There is a $50 fee paid for by the insurance company.
