UC REGENT MANDATE
As a non-academic condition of enrollment all graduate and professional students are required to maintain continuous coverage under an approved health insurance plan at all times while enrolled at UC San Francisco. All registered students are covered under the UC Student Health Insurance Plan (UC SHIP) unless they have applied and have been approved for a waiver.
APPLY FOR A WAIVER
Please complete the insurance waiver criteria worksheet beforehand to make the online application process go smoothly. You will need the following to begin the application process:
- MyAccess ID and password
- Insurance company contact information
- Insurance policy group number, member ID, and summary of benefits
Applications for New and Readmitted students for the Summer 2018 quarter are now being accepted. To apply, go to the Student Portal and click on the "Health Insurance Waiver" link. The deadline to submit an application for the Summer 2018 quarter is Friday, June 15th (11:59pm PST).
As part of your application you agree to provide a copy of your health insurance identification card or other documentation as requested by the University, its' agent or the Insurance Company. Failure to provide documentation upon request, the University will have the right to enroll you in the UC Student Health Insurance Plan and the appropriate fee will be charged to your student account.
Applications are accepted fall, winter and spring terms. Students will need to reapply each academic year by the applicable deadline for the term in which they wish to be granted a waiver. There is a $78 application processing fee. This application fee will be collected via the registration process.
Student Health and Counseling Services (SHCS) will conduct a waiver application and verification process for new waivers on the following schedule:
|Term||Applications Open||Applications Close*|
|Fall 2017||July 3||September 12|
|Winter 2018||November 13||December 21|
|Spring 2018||February 12||March 29|
|Summer 2018 (new summer and re-admitted students only)||May 1||June 15|
* Applications accepted until 11:59pm (PST)
Waivers cannot be applied mid-term and will not be processed for continuing students in summer term. Only new and re-admitted students will be allowed to apply for a waiver in the summer.
SHCS will verify waiver applications for all terms. SHCS will also verify students with waivers a second time at some point during the year to ensure all policies are still in effect.
If SHCS is unable to verify a student’s plan prior to the approval deadline or if SHCS is informed at any point that a student’s plan has terminated, SHCS may at any time re-apply the full insurance fee for the term in question.
CANCEL YOUR APPROVED WAIVER
Students may cancel their waiver and re-enroll in UC SHIP at the beginning of any term. To cancel your waiver, please fill out the Insurance Waiver Reversal Form and return it to SHCS prior the start of the term for which you would like to re-enroll.
If a student involuntarily loses their coverage (ex: hit age 26, are covered under a spouse who loses their job and health coverage, etc.) they may re-enroll in UC SHIP mid-term. The student must submit an Insurance Waiver Reversal Form to SHCS along with an official written letter of termination from the previous health insurance carrier. The student will be enrolled as of the date of their involuntarily loss of coverage if they notify Student Health within 31 days of the loss of their coverage. If the student does not notify the Student Health within the 31 days, coverage will be effective on the date the student pays the full premium. Mid-term enrollment does not allow for any proration of the premium.
Students who have access to an employer-sponsored insurance plan or an individual plan that meets all minimum benefits levels will be eligible to waive the UC Student Health Insurance Plan. Please see the insurance waiver criteria (waiver criteria are subject to change each academic year).
If a student’s application for a waiver is denied, they may file an appeal with Student Health and Counseling Services by submitting the Insurance Waiver Appeal Form. Appeals must be submitted within 10 days of the date of the denial notice.
The insurance premium will be removed from the student's fees and the application fee applied within 5 business days of the waiver application being fully processed and approved.
An approved waiver in the Fall 2017, Winter 2018, or Spring 2018 quarter is valid through the end of the Summer 2018 quarter so long as you maintain continuous coverage under your plan.
An approved waiver in the Summer 2017 quarter for new and re-admitted students is valid through the end of Summer 2018 so long as you maintain continuous coverage under your plan.
A new waiver application will need to be submitted for the 2018-19 academic year.
Even if you waive the UC Student Health Insurance Plan, you will still have access to Student Health and Counseling Services. Those services include:
If you missed the waiver deadline please email Student Health and Counseling at [email protected] to request a late waiver application. There is an $88.00 late fee. Late waiver applications are accepted 30 days following the waiver deadline.
If you have questions related to opting out of the student health insurance plan or you are a student who speaks English as a second language please contact 415.476.1281. A staff member will be happy to talk you through the process.
Students may elect at the beginning of any new term to cancel their approved waiver and enroll in UC SHIP. To do so the student must submit to the University an Insurance Waiver Reversal Form. The form must be received within thirty (30) days of the start of the quarter for the effective date of coverage to be the quarter start date. The student must pay the entire premium for the term in which they are electing to enroll.
If a student involuntarily loses coverage under their health insurance plan mid-term the student must submit to the University an Insurance Waiver Reversal Form along with an official written letter of termination from the previous health insurance carrier. The student will be enrolled in UC SHIP as of the date of their involuntarily loss of other coverage if they notify Student Health and Counseling Services within 31 days of the loss of their coverage. If the student does not notify the Student Health and Counseling Services within the 31 days, coverage will be effective on the date the student pays the full premium. Mid-term enrollment does not allow for any proration of the premium.
Students needing any care typically paid for by your insurance plan must follow the guidelines and rules of their insurance plan. SHCS is not a network provider on any insurance other than UC SHIP and our referrals may not be accepted. If you need a specialty visit, x-ray, prescription, etc. you will need a referral or order from a provider affiliated with your plan. Most times, prescriptions written by SHCS provider will be covered by your plan.
It is up to the student to work with their insurance carrier to appropriately obtain care outside SHCS when required.
The reason health insurance is a mandatory condition of enrollment is to ensure (a) you have access to medical care anytime you need it, (b) you will not experience a financial catastrophe in receiving the care you need and (c) you have the help you need to avoid compromising your academic progress.
The UC Student Health Insurance Plan is an affordable health insurance plan tailored to the health care needs of UC San Francisco students. It features year-round, world-wide coverage using the Anthem PPO network; this means students can receive convenient, high quality care when they are sick or injured. Included in the student insurance plan is dental coverage through Delta Dental and vision services through Blue View Vision Care. The UC Student Health Insurance Plan provides comprehensive coverage for all your healthcare needs.
The UC SHIP is designed to "complement" the services offered at Student Health and Counseling. It provides benefits including emergency/after hours care, hospitalization, surgery, specialized care not provided at Student Health and Counseling, specialized testing, e.g. CT scans or MRI's. It would be ideal if your private plan did the same, without you having to leave the campus area.
Please see the links below highlighting the medical, dental, vision, and prescription benefits of the UC Student Health Insurance Plan. Compare to your alternative plan to see what works best for you and your family.
